It is a trapping hazard or may electrocute people in the health care setting.
You can control this hazard by unplugging it or if not possible tape the wire to the wall or floor out the way.

It may cause scratch or burn marks by walking through them.
You can control this hazard by ether pinning back the plastic flaps when people are going in and out the door or take the plastic flaps down.

There are two ways the children can be at risk these are:

The first is unsanitised toys can increase the risk of cross infection.

And the second is broken toys can cause injuries such as a cuts and bruises.
You can control the risk of cross infections and injuries by sanitize toys and remove any broken toys.

There is a risk of people falling and hitting their heads on the corner of the table causing all different head injuries from just a cut or bruise on the head to being unconscious.

You can control the risk of people falling and hurting their heads by ether sandpapering the edges of the tables and putting rubber or any soft material on it or get round tables

There is a risk from slipping on the wet floor and causing injurers
You can control this by putting up wet floor sign and cleaning it up as soon a possible

By having the chair to small for the desk can strain the child's eyes and affect their posture and cause back injuries.

To reduce the risk from this happening you need to get a bigger chair or a smaller table

Chemicals e.g. Bleach, toilet cleaner, washing up liquid etc...
Children are at risk from poisoning, skin and eye irritation.
The kitchen gate must be closed at all times and any chemicals looked securely in a cupboard with a Lock.

Children are at risk from trapping their fingers which might cause brushed or broken fingers.

To reduce the risk of trapping the child's Fingers in door hinges all must be aware of the children around the hinges and fit protection strips on door hinges

Children are at risk of being electrocuted if they stick any thing in the socket e.g. Toys and fingers
To reduce the the risk of the child being electrocuted you will have to make sure you put safety plugs in all sockets

The first column (L) is for an assessment of the likelihood of the hazard taking place, the second column (S) is for the severity of the hazard. The assessor allocates a number to the (L) & (S) columns. These are then multiplied together to produce a Risk Level between 1 and 9 which is then entered into the corresponding (R) Colum.

2. Likely to occur; hazard exists intermittently or occurs occasionally
3. Likely to occur soon; permanent hazard or occurs daily/repeatedly; many may be exposed
