Section 1: Understanding the holistic approach to end of life care

1) Various needs that would be considered when planning an individuals end of life care;

a) The physical needs to be considered for a person needing end of life care are likely to be pain, nausea and vomiting, breathlessness and weakness/fatigue. Some people may also need help with mobility and personal care.

b) The emotional/pysychological needs to be considered for a person needing end of life care are anxiety, fear and depression as these are very common towards end of life and can change very quickly. Emphatic support, understanding and reassurance

'PEPSI COLA' Aide Memoire Holistic Checklist - This is a tool that aims to capture a persons hollistic needs and is used as part of the Gold Standards Framework.

Problems Checklist - This is used to assess the prevalence and severity of physcological problems experienced by people with cancer.

Sheffield Profile for Assessment and Referral to Care (SPARC) - This is used to assess the distress caused by advanced illness and to screen symptons and problems in order to guide referrals to specialist palliative care services.

Symptons and Concerns Checklist - This is used to determine the prevalence and severity of symptons and concerns alongside a clinical assessment.

4). A range of tools that can be used to inform the assessment process and what aspect each tool would commonly be used for are: Karnofsky Performance Status score - These can be used to measure the persons performance in a number of key areas over time. They can help proffesionals to assess deterioration of abilities and predict time remaining.
