Bi There are many ways in which you can find out information on an an individual using your service. You can get details on their history, preferences wishes and needs by referring to:
1. Person centred care plan - This should have details of their wishes and preferences along with how much support is needed for them on a day to day basis. You could also find out their likes and dislike, these should also be listed in their personal care plan along with possible triggers of the individual.
2. Asking The Individual - You could also ask the individual of there likes, preferences and their history if they feel comfortable to talk about and answer your questions. Also if the individual is enabled, if the individual is not abled to provide you with the information that you require you could try and gain the history from previous places of residence, family member or contact and advocacy.

Bii There are many ways in which you can make sure that information on an an individual using your service is up to date this will include their history, preferences wishes and needs you can do this by:
1. Updating care plans - Making sure that care plans are up to date, all the information that a support worker will need to provide the correct level of support is within the care plan and is easy accessible. Care plans should be updated and reviewed when: the correct care plan is not meeting the individual's needs, if anything medically changes with the individual or at least every 6 months.
2. Meetings - Having meetings and reviews with the individuals, or the individuals representative/ Advocate; this could allow you to check that the individual is happy with their care plan, it is meeting their personal needs and wished and that the care plan is based on their personal opinions and values

Having a person centred care plan helps social care workers to work in a person centred way through a variety of reasons. Working to a Person centred care plan ensures that the individual is happy with their care that they are receiving, that it is meeting their personal needs and wishes,that the care plan is based on their personal opinions, values and that they have a direct influence on the care that they receive. A person centred care plan allows you to provide the best care for an individual in a way that they will feel comfortable.

The term active participation means to have a certain approach that will enable individuals to be included in their care. To have a say in how they are cared for, what they would like to do and what help and support they would like to receive along with much more other aspects. It is good for an individual to have a say on things that will go on in their life as we all have the same human rights to make decisions for ourselves. Individuals having an active participation may help them to: increase activity and exercise levels
Increase their independence and basic life skills
Help to create better working relationships between carers and service users

An individual helping to plan their weekly activity planner gives the an active participation in how they receive their care and also ensures that they receive their wishes and that there preferences are taken into account this will benefit them by:
Feeling empowered, more independent and increase their self esteem by getting to choose their own activities and help create something that they are going to do and goals to achieve.
Feel settled knowing that they have events, activities and other things that they have planned to look forward to, they don't have to worry about not being able to go out or not doing an activity which they enjoy.
Makes sure that they are having a direct influence in their planned task for the week and this could reduce the possibility of challenging behaviors

An Individual choosing something that they would like to learn, weather that is a new skill such as cooking, cleaning or even how to brush their own hair.
