Brandon - Kindergarten
1. What behavior problems is the student exhibiting? If more than one, which one should be addressed first?
Student struggles with sitting still, makes noise frequently, will not speak unless spoken too, does not like loud noises.
First to be addressed is the inability to sit still. Once able to sit still student might be able to listen and follow directions.

2. Where is the problem occurring? Is there any indications of how long it has it been occurring?
The problem is occurring at school. No know length of time for behavior.

3. What is the probable cause of the student’s behavior? What information do you have to support this?
Cause appears to be Autism. Struggles to sit still, makes car noises, will not make eye contact, non-verbal most of the time, advanced knowledge of computers, does not like loud noises.

4. What positive behaviors should be reinforced to replace the problem behavior?
Sitting still and keeping hands and feet to self should be reinforced when student shows difficulty sitting still.

5. What should be done when the child exhibits the problem behavior? What about the positive behavior?
Problem behaviors should be redirected and reminded of expectations. Positive behaviors should be rewarded with computer time.

6. What other concerns, if any, need to be taken into account when preparing the behavior plan for this student?
Student is sensitive to loud noises and has trouble completing work

7. What strategies can be used to deal with and modify, the student’s problem behavior ?
Behavior charts, stickers, computer time.

Christopher – first grade
1. What behavior problems is the student exhibiting? If more than one, which one should be addressed first?
Possible depression and lack of confidence. Should address depression first.

2. Where is the problem occurring? Is there any indications of how long it has it been occurring?
The problem is occurring at school. Has been going since accident over summer vacation.

3. What is the probable cause of the student’s behavior? What information do you have to support this?
The car accident. Was top student and now struggling with academics.

4. What positive behaviors should be reinforced to replace the problem behavior?
When student is doing work give positive feedback.

5. What should be done when the child exhibits the problem behavior? What about the positive behavior?
Get students attention and offer to sit with student and give assistance with schoolwork. When showing positive behavior offer rewards or encouragement.

6. What other concerns, if any, need to be taken into account when preparing the behavior plan for this student?
Lack of self-esteem and sadness

7. What strategies can be used to deal with and modify, the student’s problem behavior?
Giving constant positive feedback
Will – third grade
1. What behavior problems is the student exhibiting? If more than one, which one should be addressed first?
Student gets frustrated when asked to read. Refuses to participate in class. Behavior to be addressed first should be controlling his frustration.

2. Where is the problem occurring? Is there any indications of how long it has it been occurring?
Problem is occurring at school and home. No indication of how long student has had visual processing disorder.

3. What is the probable cause of the student’s behavior? What information do you have to support this?
Probable cause is visual processing disorder,student has a diagnosis.
4. What positive behaviors should be reinforced to replace the problem behavior?
When student is reading, doing school work and not shutting down.

5. What should be done when the child exhibits the problem behavior? What about the positive behavior?
Giving student time to get self together. Maybe even a little time out to gather thoughts. Positive behavior could be rewarded with free time to read silently.
