Six Dimensions of Health Worksheet

Part 1

For each of the following six dimensions of health, list at least one characteristic, activity, belief, or attitude that reflects that dimension in your life. Provide a brief explanation with each example. Refer to Ch. 1 in the text for explanations of these dimensions.

Physical health: I love running. This is the activity that I associate most closely with my physical health. I feel that running increases my physical health. I feel great about myself and body after a run.

Social health: At work is where I build my social health. I work with a lot of different people on a daily basis. Meeting and communicating with these people makes me feel good about my social health. I feel that the more I talk and interact with new people the better my social health is.

Intellectual health: I practice this at work as well. I use my brain frequently to find different ways to complete tasks. Having to find the right people to complete tasks takes a lot of brain power. By doing this on a daily basis I am building my intellectual health.

Environmental health: Environmental health is one of the dimensions I’d like to work on. I feel that I do my part in not littering which preserves the environment. I also don’t cut down trees and wreck the environment. But I feel that I could do more. I don’t recycle as much as I should, that is one thing I’m trying to change.

Emotional health: I use emotional health every day. I am self-confident in everything I do. Whether it’d be playing sports or getting a task done. When it comes to doing something, I know that I will give it my all and complete it. I enjoy trying new things and pushing myself. My self-efficacy is great. I know I will complete any task put in front of me.

Spiritual health: Feeling as if I was put on this Earth for a reason is what gives me good spiritual health. For me, I was put on this Earth to help people. Whether it’s helping my little sisters in homework, or
