Identify educational needs:
*The Sanchez family needs education about obesity.
*Proper and balanced nutrition.
*How to incorporate physical activities into their daily living.
*Diseases related to obesity.
Educational goals:
The Sanchez family will verbalize understanding of the importance of physical activity.
The Sanchez family will become more physically active.
The Sanchez family will have a better understanding of the risk associated with obesity and poor nutritional choices.
The Sanchez family will have a better understanding of the importance of healthy food choices for a healthy lifestyle.
Objective:
By May 20, the Sanchez family will involve in safety steps to decrease overweight, and will make exercising a safe routine family activity.
Appropriate educational method: Brochure.
Evaluation:
At the end of presentation, the Sanchez family verbalized understanding regarding the importance on decreasing overweight, incorporating physical activities into their daily routine, and healthy nutritional choices to prevent obesity and its sequelae.

Self-evaluation:
I presented to the Sanchez family a brochure educating them about obesity, benefits of physical activity and how to change eating habits to decrease overweight.
