Please describe in detail the extenuating circumstances that have led to your dismissal as well as any plan of action you plan on implementing in the future to avoid academic difficulties.
To Whom this may Concern;
I, Tracy Woodson-Mills, am writing this letter to request to be reinstated into school at South University
Online class. I was academically removed from school because I did not meet the standard requirements for the school. I failed to pass Human A & P class within three (3) attempts. During the class attempts, I had a lot of personal and financial situations going on at the time.
During the first start of class my mother became very ill, so I traveled from Chicago to Mississippi to see her. On June 12, 2012, my mother passed away and that was a very emotional time for me. I requested a two (2) week leave from work so that I could stay in Mississippi and handle some unfinished business on my mother's behalf. While at her house there was no computer access for me to complete the assignments. Being in a very rural area the library was only open on certain hours and days, so when I did get a chance to get to town I complete some assignments. The assignments was late, but I did complete them, but it was not enough to pass the class.

14, 2012 I received a call stating that my uncle had a major stroke and was on life support. After my mom passed, I became his power of attorney. I had to request some time off from work to travel back to Mississippi to take care of his medical concerns. At this time I contacted my counselor which was K. Ziegler , to request to be withdrawn from the class. I knew that with all the traveling back and forth that I would not be able to focus on my studies. My uncle was in the hospital from July to September 2012. After he was released from the hospital, I staying in Mississippi until October to make sure that he was cared for properly. In October, I place my uncle in a rehabilitation institution and came back to Chicago to get back to work and school.
I knew that I would have to pass this class, because I thought it would be my second attempt. But, once I asked my counselor to start the class again I noticed that the second attempt was a withdraw with a fail. I asked her to explain this grade and she stated that I did not withdraw within the time that was allowed. At the beginning of the class I did not have the Adobe Professional X needed to complete some of
