Dear Craig,

I am writing to you with reference to the recent incident that took place in the organization, in Brisbane. The incident, OHS incident, has raised a number of concerns and has highlight some of the essential and significant short comings of the organization, that need to be discussed at high level and preventative measures should be calculated, in order to avoid any such incident in future. As the incident deals with the health and well being of the employees, it has raised a number of concerns regarding the ethical rules and regulations of the organization, and thus the reputation is on stake.

It is eminent to mention here that the incident took place mainly because of the previous problem of the employee, and as mentioned in his file, he has been suffering from the issue since25 years, but has somehow managed to work along with the pain. Although he adopted various methods to minimize pain and keep the work unaffected, the lengthy repair jobs were not suitable for him and keeping in view his condition, he should have retired on the onset of problem. It should be noted that Gray was aware of the working conditions in the company and should have chosen between the job and his health. Incident took place with Gary was certainly a reminder of serious flaws in our existing occupational health and safety procedures. I believe incident took place because of following reason:

Employees are performing repetitive task for long hours in standing positions
Work bench is bit too high and its making task more difficult
Seat in the van is uncomfortable to sit for long periods
Employees are not aware of correct way of performing task to avoid injuries.

Above reason indicates that’s we need make immediate changes in our OHS procedures to avoid any future injuries. According to OHS legislation it’s mandatory for all employers to make sure their employees are working in healthy and safe environment. Therefore suggest following changes to our workplace:
1. Ensure all the workers take short rest
